How These Services Are Covered by Medicare
Preemptive provides care through several long-standing Medicare programs designed to support people with ongoing health needs.
These programs exist because research and experience have shown that continuous monitoring and care coordination reduce emergency visits and hospital stays.
Depending on your individual needs, your care may be supported through one or more of the following:
Remote Health Monitoring (RPM)
This program supports the collection and analysis of health data while you live your normal life at home. By seeing trends over time, clinicians can often address problems early, before they require urgent care.
Ongoing Care Coordination (CCM)
This program covers the time nurses spend reviewing information, adjusting care plans, communicating with providers, and helping you stay organized. It ensures that your care remains connected and consistent.
Specialized Support (PIN)
For members with more complex conditions, this program provides additional guidance and coordination across specialists, medications, and follow-up care.
Community & Lifestyle Support (CHI)
This program helps address practical factors--such as access to services, transportation, or support resources--that influence long-term health.
All of these are established nationwide Medicare benefits. They are part of routine healthcare--not experimental programs.

Hospital care is the most expensive part of healthcare. When problems are caught early, many hospital visits can be avoided. Medicare supports programs like Preemptive because they improve health outcomes and reduce long-term costs.
No. Preemptive is a fully licensed medical clinic providing standard medical services. Members receive clinical care--not research participation.
No. This is a medical care program, not a compensation or survey program. In some outreach efforts, a small gift card may be offered for an initial survey, but there is no ongoing payment.
For most members, out-of-pocket costs are nothing. We review your specific coverage ahead of time and walk you through it clearly, so you always know what to expect before getting started.
Yes. Participation is always voluntary, and you may discontinue at any time.
Who This Program Is Designed For
Preemptive supports adults who want more consistent guidance managing their health--especially those living with chronic conditions or age-related risks.
Because services are covered through Medicare programs, eligibility must be determined by a clinician. In general, this includes:
- One or more ongoing health conditions expected to last at least 12 months
- A documented intake, wellness visit, or assessment
- Your informed consent
- Coverage and licensure that apply in your state
